Continuously adding weight and reps beyond 6-8 weeks suggests starting too far from failure. Intermediate and advanced lifters training genuinely hard typically progress for 4-8 weeks maximum before stalling.

Starting mesocycles at two or three reps from failure, then progressing to actual failure by the final week, creates ideal stimulus while managing recovery demands.
