For practical purposes, think of peptides as chemical messengers that help regulate bodily processes including metabolism, inflammation, growth, and repair.
Insulin serves as a perfect example. It signals muscle and fat cells to absorb sugar from the bloodstream while instructing the liver to store glucose and reduce output. This maintains blood sugar within optimal ranges and ensures appropriate glucose utilization for energy.
We understand insulin’s mechanisms extremely well because it has undergone extensive study in laboratory models, animals, and crucially, humans—all part of the rigorous FDA regulatory review required before substances can become prescription medications in the United States.
From Bodybuilders to Biohackers: The Peptide Boom
The wellness peptide phenomenon started in the mid-2000s when bodybuilders began experimenting with these compounds for competitive advantage. Interest gradually spread to biohacking communities before fully exploding once GLP-1 medications consumed the media cycle.
This gave many people hope that peptides might genuinely possess transformative powers for virtually every health concern imaginable.

However, obtaining these wellness peptides wasn’t straightforward. In 2023, the FDA identified significant safety concerns involving many compounded peptides, prompting users to get creative.

Many began acquiring peptides through telemedicine platforms, longevity clinics, functional medicine practices, med spas, or even gray-market websites selling them as “research chemicals” or “not for human use.”
The Underground Market and Its Dangers
This created loopholes where regulation couldn’t keep pace. Some clinics arranged prescriptions through compounding pharmacies, but numerous online vendors sold directly without any medical supervision whatsoever.
While some peptides were applied topically or taken as capsules, many came supplied in vials for self-injection under the skin.
People were injecting chemicals into their bodies that had zero regulatory review, no oversight, and were likely manufactured abroad by questionable companies with potentially dangerous contaminants.
To put this in perspective: COVID-19 vaccines underwent exponentially more testing and scrutiny by scientists globally. If you felt those vaccines were rushed or approved too quickly, the premature adoption of these peptides should be genuinely alarming.
Celebrity Endorsements Fuel the Fire
What made this trend particularly frustrating was celebrities constantly appearing on podcasts to share their peptide “stacks” and rave about results.

These understudied and sometimes fully disproven compounds were being promoted with little pushback, creating an information war against medical professionals who urged caution.
One physician experienced this firsthand after appearing on Fox News to discuss peptides. When highlighting that current evidence doesn’t support many popular claims about injectable peptides for muscle healing, tendon repair, or gut health, backlash was swift.
Ironically, many people who oppose medications like Ozempic and accuse doctors of being “drug pushers” became angry at physicians wanting more safety data before recommending peptides. The hypocrisy is real.
The Shocking FDA Advisory Committee Vote
Given this landscape, what should have happened next seemed straightforward: peptides would undergo human studies, safety data would be released, and medical professionals could begin recommending proven compounds for appropriate conditions.
But then Robert F. Kennedy Jr. entered the picture as HHS secretary. Before his appointment, he publicly promised to end what he called the FDA’s “aggressive suppression” of peptides, having personally used and believed in them.

Kennedy encouraged the PCAC (Pharmacy Compounding Advisory Committee) to conduct a new review of peptides. But this wasn’t an evaluation of FDA approval—it was about whether certain peptides including BPC-157 and TB-500 should be added to the FDA’s 503A bulks list.

Understanding the 503A Bulks List
This list allows compounding pharmacies to legally make medications for patients using raw bulk ingredients that have never been FDA approved.
Being on this list does not mean FDA approval for general use. It means licensed pharmacists or physicians can use these substances to create customized medications without full FDA review—intended to support legitimate compounding for unique patient scenarios, not as a shortcut bypassing standard approval processes.
Four questions determine whether substances belong on this list:
- Can the substance be reliably identified and manufactured?
- What safety concerns does it raise?
- Is there evidence it actually works?
- Is there meaningful history of use in compounded medications?
What FDA Scientists Actually Found
The committee reviewed seven popular peptides. Before meetings, FDA scientists spend months evaluating chemistry, animal research, human data, administration routes, and potential risks.
Their findings were startling.
For BPC-157—probably the most famous peptide—the agency found only one small human study: a meeting abstract (not full publication) involving 53 people where it was administered as an enema for ulcerative colitis. They deemed this inadequate to establish effectiveness or safety.

The FDA found no randomized controlled human trials establishing safety or effectiveness of injecting BPC-157 for tendon, ligament, or muscle recovery.
For KPV, TB-500, and Matsepa, the FDA reported no human exposure data could be identified at all. Scientists couldn’t find adequate published evidence showing what happens when these substances are administered to humans.
Manufacturing and Quality Concerns
Beyond efficacy questions, basic manufacturing issues arose. Peptides can aggregate, degrade, trigger immune reactions, or contain impurities.
When chemicals are synthesized, shipped as powder, reconstituted, and injected, small differences in purity, storage, or formulation matter tremendously. Even if animal studies used “BPC-157,” that doesn’t guarantee every vial sold online or bulk ingredient delivered to pharmacies is chemically identical to what was studied.
Exact TB-500 sequences can vary between vendors. When you write a script for TB-500, what form do you think your patients are getting? Does anybody know?
FDA scientists made their conclusion: they recommended against adding every single peptide to the 503A bulks list. All seven got no recommendations—not because peptides were proven useless or dangerous, but because evidence wasn’t strong enough to confidently determine what patients would receive, whether it would help, appropriate dosing, or potential harmful effects.
The Committee Voted Against FDA Scientists
The public meeting proceeded with FDA scientists presenting analysis, supporters and opponents testifying, and committee members asking questions. Then came the votes.

The strongest pro-peptide argument wasn’t that substances were proven safe or effective—it was harm reduction. Supporters argued Americans were already buying and injecting peptides regardless of FDA positions, ordering from overseas suppliers and websites labeling products as “research chemicals.”
Just last week, I had a patient whose research grade whatever that they got from some place that they brought to me to fix was laced with MDMA and ecstasy. That’s not protecting the American people.
Their logic: not including peptides on the bulks list pushes demand toward even less controlled gray markets. If people will use unproven substances anyway, obtaining them from licensed pharmacies might be less dangerous than injecting mystery powders purchased online.
While somewhat logical, this reasoning doesn’t answer what the approval process is designed to determine: usefulness, safety, and proper manufacturing standards.
The Stunning Results
Then came BPC-157’s vote.
For the record, there are eight yeses, six nos, and one abstention.
The committee voted to recommend BPC-157 over FDA scientists’ opposition. KPV and TB-500 received the same eight supporting votes. Matsepa narrowly passed. By meeting’s end, the committee recommended adding six of seven peptides despite scientists recommending against every single one.
The Conflict of Interest Problem
This didn’t immediately legalize peptides—the committee functions in advisory capacity. But how did this happen? Why did scientists recommend against all seven while the committee voted to recommend six?

Who was voting reveals the shocking truth.
Eight of the committee’s 11 regular voting members had begun terms just weeks before the meeting. The committee was essentially rebuilt right before considering one of RFK’s priority projects.
According to reports, FDA officials including Dr. Marty Makary raised concerns about potential conflicts, but HHS moved forward with selections anyway. Dr. Makary later resigned under threats of termination.
Who Were These New Members?
The eight newly appointed members included owners and executives of regenerative medicine, longevity, men’s health, and personalized medicine clinics, along with professionals connected to pharmacy and compounding businesses.
Several independent reports identified potential financial conflicts because their businesses operated in areas that could benefit from expanded peptide access. They worked at clinics offering, promoting, or operating in the same longevity marketplace that stood to gain if FDA permitted these substances to be compounded.
One member was a pharmacist connected to a compounding pharmacy and also the son of Representative Diana Harshbarger, who previously urged Kennedy to convene this exact committee review.

Despite concerns, no conflict of interest waivers explaining why these members could participate were announced.
The voting pattern was predictable: every single pro vote for any peptide came from these eight new members. Every pre-existing expert voted against or abstained from adding peptides for BPC-157, KPV, and TB-500.
A Dangerous New Precedent
What’s unfolding isn’t groundbreaking research proving peptides help patients—it’s a potentially dangerous shortcut for medication approval.
Instead of requiring companies to conduct trials and earn approval for specific medical uses, this approach could allow experimental substances to reach patients without proving safety, efficacy, or quality.
This creates perverse incentives. Companies may no longer need to invest time and money studying medications. If influencers, clinics, and online communities create enormous consumer demand first, they can point to that demand as evidence that access is necessary.
That process is backwards. Instead of funding high-quality research to support claims, companies profit from demand rather than proof.

For all pharmaceutical industry problems, at least companies must prove medications work, establish proper dosing, and demonstrate safety profiles. This scenario potentially creates a worse version—one where no proof is needed, money flows freely, and corruption runs rampant.
The Bottom Line on Peptides
Could some peptides eventually become useful medicines? Absolutely. Insulin was once experimental. Semaglutide was once experimental. Every medication begins as an unproven hypothesis.
But promising potential and podcast excitement represent only the beginning of a rigorous process. The vast majority of treatments with initial promise don’t pan out.
Remember thalidomide? That medication caused devastating birth defects globally. The reason it didn’t come to America was because the FDA denied it—exactly the kind of regulatory protection that’s being undermined by shortcuts and conflicts of interest.
The peptide story isn’t over. But what happened at this advisory committee meeting should concern anyone who values evidence-based medicine, transparent processes, and protection from unproven substances marketed through hype rather than science.
