Scheduling Changes Feel Like Moving Mountains
At normal body fat levels, minor schedule changes are no big deal. But at 6%? Everything feels overwhelming.
When I have my day planned out, anything that alters it is usually at normal body fat levels, 10% plus. Like, okay, cool. No worries. Make the change. At 6%, it is such a struggle to just continue to put one foot in front of the other and everything feels so overwhelming.
Dr. Mike describes even minor adjustmentsโlike filming an hour earlierโas requiring monumental mental effort. His bandwidth becomes so narrow that predictability becomes essential for survival.
Energy levels drop so low that any additional demand feels massively over capacity, like being asked to sprint when you’re already jogging at maximum effort.

Big projects, long hikes, even multi-hour drivesโtasks that normally seem routineโsuddenly feel impossible at 6% body fat.

Sex drive at this point is mostly theoretical. It’s weird to handle sexual needs as something you just got to check off the list every other day in the shower staring thinking what can I think about? Food? Nope.
From an evolutionary perspective, this makes sense. When your body perceives starvation, reproduction takes a backseat to survival. Attraction and desire become shadows of their normal intensity.
Attention Span Narrows to Survival Basics
Dr. Mike’s entire mental aperture shrinks to just six things: food, training, work, sleep, steps, and evening TV.
Anything outside these basicsโart, museums, exploration, even conversationโstruggles to register as interesting. Vacations become pointless because bandwidth for leisure activities simply doesn’t exist.
He traveled to Singapore and Melbourne during this periodโnormally exciting destinationsโbut experienced them in “zombie mode” with minimal engagement.
Sleep Requirements Increase Dramatically
While Dr. Mike normally functions great on 7.5 hours of sleep, maintaining 6% body fat requires 9 full hours to feel remotely functional.
His body essentially demands extra recovery time to manage the physiological stress. Sleep quality remains goodโpartly aided by growth hormoneโbut quantity becomes non-negotiable.
Critical point: If sleep quality collapses entirely, the diet should probably end. Sleep is too fundamental to fat loss, muscle retention, and health to compromise.
Muscle Pumps Disappoint Despite High Carbs
Even at maintenance calories with 400g carbohydrates daily, muscle pumps remain mediocre at 6% body fat.
Dr. Mike’s best pumps occur around 10% body fat when properly carb-loadedโcomplete with vascularity and fullness. At 6%, pumps exist but feel underwhelming despite adequate nutrition.
Training Feels Terrible… Until It Doesn’t
Here’s where things get interesting for lifters trying to maintain muscle while extremely lean.
Every training session starts with Dr. Mike thinking “there’s no way I can do this.” Warm-ups feel terribleโlike starting a rusty car. First working sets are mediocre at best.
But then something changes.
A few working sets into the workout, I start to feel pretty good. My sympathetic nervous system, fight or flight, starts to kick in.
About 20 minutes in, after a couple working sets, fight-or-flight activation kicks in. Suddenly training feels manageableโeven good.
Critical Training Tip: Don’t Reduce Volume Early
Never cut training volume based on how you feel at the start of a workout.
Because you’ll feel terrible and reduce sets unnecessarilyโrobbing yourself of growth stimulus. Wait until you’re 1-2 working sets in. By then, you’ll likely feel capable of completing your full program.
If halfway through you’re genuinely struggling? That’s when to make adjustmentsโeither by cutting the workout entirely, taking a deload, or increasing calories temporarily.
Mood Flattens and Social Energy Evaporates
Three hours post-meal, mood doesn’t just sourโit flattens completely.
Irritability increases, but even irritation requires energy Dr. Mike doesn’t have. He describes himself as “a dying possum that still hisses at passersby.”
Everybody I see I hate. And I really mean that. I hate not just people. I hate dogs. I hate cars. I hate skyscrapers.
Talking, texting, even taking photos to share with friends becomes too effortful. Dr. Mikeโnormally extremely talkativeโbecomes quiet and withdrawn.
People meeting him for the first time in this state probably think he’s introverted, when reality is he’s just severely energy-depleted.
Practical Survival Tips for Extreme Leanness
Based on his experience, Dr. Mike offers specific strategies for anyone attempting to maintain very low body fat:
Remember It’s Temporary
Don’t internalize this state as your new identity. You’re not permanently mean, introverted, or low-energyโthis is situational and reversible.
Reduce Daily Tasks to Essentials
Strip your schedule down to training, eating, steps, and recovery. Don’t pack vacation itineraries. Avoid scheduling multiple activities daily.
Chilling isn’t optionalโit’s essential.
Skip Vacations Unless You’re Eating in Surplus
Active vacations with walking tours and sightseeing won’t work at extreme leanness. You simply won’t have bandwidth to enjoy experiences outside basic survival mode.
Carry Bars and Shakes Everywhere
With depleted glycogen stores, you’ll need frequent refuelingโnot just for muscle growth but for basic energy to walk and think clearly.
Practice Self-Compassion and Kindness
You’ll get irritable. Negative thoughts will arise constantly. Recognize these as symptoms of your physiological state, not reality.
You’re doing this shit voluntarily, remember? Keep the shit to yourself. You got nothing nice to say, shut up.
Consider Retatrutide for Energy

The only advantage to staying low body fat for longer is that you look cool. The only thing better than looking lean now is setting yourself up to have more muscle to look even leaner and more jacked later.
