A potentially revolutionary breakthrough in aging science just moved from the lab to human trials.

For the first time ever, researchers are testing a therapy that could actually reverse biological aging—not just slow it down.

Harvard geneticist Dr. David Sinclair, who’s been working on this technology for years, recently announced that his age-reversal treatment has received FDA approval to begin clinical trials in humans.

In a recent podcast conversation with James Altucher, Sinclair revealed the science behind this groundbreaking approach and what it could mean for treating everything from blindness to Alzheimer’s.

The Three-Gene Solution That Changes Everything

Back in 2019, when Sinclair published his bestselling book Lifespan, the concept of reversing aging was still theoretical. Scientists weren’t even sure if it could be done safely without triggering cancer or cell death.

The breakthrough came from an unexpected source: Yamanaka factors, a set of four proteins discovered by Nobel Prize winner Shinya Yamanaka for creating stem cells from adult cells.

If you want something simple that actually works, this is one of the most effective tools I’ve used to build strength, conditioning, and endurance without needing a full gym setup.

  • Full-body training with one weight using swings, squats, and presses
  • Solid cast iron build that feels stable and lasts for years
  • Comfortable grip that makes high-rep workouts easier to handle

In retrospect, it seems obvious to people who just hear about it that you just do this and age is reversed. But back when I was talking to you in 2019, we it was still early days and we really didn’t know if it was possible to safely reverse aging or if every time it would lead to death or cancer.

Sinclair’s team discovered something remarkable: using just three of the four Yamanaka factors—excluding the one most likely to cause cancer—could reverse cellular aging by approximately 75% without triggering dangerous side effects.

The cells stopped aging before reaching a complete “factory reset” that would erase their specialized functions and potentially cause tumors.

Why Start With Eyes?

Sinclair’s company, backed by Life Biosciences, chose glaucoma and eye damage as the first conditions to treat in humans for strategic reasons.

The eye offers a contained testing environment where the therapy can be localized, making it safer and easier to monitor. More importantly, glaucoma causes irreversible vision loss—there’s currently no cure.

It does feel very significant given that there’s never been anything that could truly reverse aging safely until now.

In animal studies, the results were nothing short of stunning. Old mice with damaged optic nerves regained their vision. Monkeys with glaucoma-like conditions showed dramatic improvements.

The therapy didn’t just halt progression—it actually reversed damage that conventional medicine considers permanent.

How the Treatment Actually Works

Patients receive an injection containing three specific genes (called OSK for short) delivered via a virus-like particle directly into the eye.

These genes don’t permanently alter chromosomes like traditional gene therapy. Instead, they’re accompanied by a molecular switch that responds to a common antibiotic, doxycycline.

Here’s the treatment protocol Sinclair envisions:

  • Weeks 1-8: Patient takes prescription antibiotic to activate the age-reversal genes
  • During activation: Damaged optic nerve cells revert to younger biological age
  • After 8 weeks: Stop antibiotic; genes switch off automatically
  • One year later: Treatment can be repeated if needed

In mice, leaving these three genes activated indefinitely caused no cancer whatsoever. Vision continued improving the longer the treatment remained active.

Beyond Vision: Reversing Brain Aging

While eye trials grab headlines, Sinclair’s lab has been testing this approach on virtually every major organ system in animals.

Perhaps most remarkably, they’ve reversed age-related cognitive decline in mice.

If you take an old mouse that has forgotten how to remember things, it can’t even learn anymore. Not only are we able to reverse that and cure it of Alzheimer’s and just essentially old age dementia, but we also saw that those mice had signs that they were remembering things from their youth again.

This suggests something profound: memories aren’t necessarily lost with age—they just become inaccessible.

If this translates to humans, it could revolutionize treatment for dementia and Alzheimer’s disease.

Other Conditions Successfully Treated in Animals

  • Multiple sclerosis: Nerve damage reversed
  • Fatty liver disease: Liver cells rejuvenated
  • ALS: Motor neuron function improved
  • Joint degeneration: Cartilage rebuilt
  • Skin aging: Wound healing accelerated

Independent research teams have now replicated similar results across brain, kidney, skin, and other tissues.

The Information Theory of Aging

Sinclair’s approach stems from what he calls the “information theory of aging”—the idea that we don’t age because cells wear out, but because they lose the ability to read DNA correctly.

Think of your genome as sheet music and your cells as orchestra musicians. When you’re young, everyone reads the music perfectly and plays in harmony.

As you age, the sheet music gets scratched, folded, and coffee-stained. Musicians start playing wrong notes. The symphony descends into chaos.

Traditional medicine treats individual wrong notes—high blood pressure here, arthritis there. Sinclair’s approach is different: clean the sheet music so cells can read it correctly again.

Make the body young enough and it will cure itself. We don’t need to target the symptoms, we target the cause.

This explains why the three-gene therapy appears to work across so many different conditions. It’s not treating diseases individually—it’s restoring the cellular machinery that prevents disease in the first place.

Timeline: When Could This Be Available?

The FDA approved trials in January of this year. Patients are currently receiving treatment or about to begin.

This Phase 1/2 trial primarily checks safety but will also measure vision improvements. Because glaucoma has no existing cure, the FDA could potentially fast-track approval if results look promising.

Sinclair’s projection: 3-4 years minimum before doctors can prescribe this for eye conditions, assuming everything goes well.

But his team isn’t waiting to move forward. They’re already preparing trials for other organs:

  • Full-body training with one weight using swings, squats, and presses

  • Comfortable grip that makes high-rep workouts easier to handle
  • Liver: Potential trials by end of 2024 or early 2025
  • Brain delivery: Engineering molecules that cross blood-brain barrier
  • Systemic treatment: Working toward intravenous delivery that reaches multiple organs
  • Pill form: Ultimate goal to replace expensive gene therapy with oral medication

The Cost Challenge

Current gene therapies often cost hundreds of thousands of dollars because they require strict manufacturing conditions and extensive quality control.

Sinclair’s lab has spent six years working to bring costs down from hundreds of thousands to potentially just hundreds of dollars once pill formulations become available.

What You Can Do Right Now

While breakthrough therapies progress through trials, Sinclair maintains that lifestyle factors significantly impact biological aging—and you can measure the difference with blood tests.

His personal protocol, refined over 15 years, includes:

Supplements Sinclair Takes

  • NMN (Nicotinamide Mononucleotide): Boosts NAD+ levels, supporting enzymes that control gene expression
  • Resveratrol: Activates sirtuins, proteins that protect DNA
  • Metformin: Prescription diabetes drug that lowers blood sugar and may extend lifespan
  • Nattokinase: Enzyme from fermented soybeans shown to support cardiovascular health

Sinclair alternates metformin with berberine, a natural compound with similar glucose-lowering effects that doesn’t require prescription.

Diet and Lifestyle

After testing extensively, Sinclair discovered that reducing meat, cheese, and alcohol dramatically lowered his inflammation markers—even though he thought he was already healthy.

My partner Serena Poon told me just try not eating meat and cheese and drinking wine every day and try that for a month to see what would happen. And she was right. My inflammation went down dramatically to levels that I’d never seen before.

His current approach:

  • Mostly plant-based with occasional fish
  • Intermittent fasting
  • Minimal to no alcohol
  • Prioritizing 8 hours of sleep (critical for brain detoxification)
  • Regular exercise

At 56, Sinclair reports having minimal wrinkles, no gray hair, and good eyesight. His 86-year-old father, following similar protocols for decades, has “nothing wrong with him”—which Sinclair notes is virtually unheard of.

The Next Generation: Beyond Yamanaka

Sinclair revealed that his lab has moved beyond Yamanaka factors entirely.

Using artificial intelligence and molecular ingenuity, his team has identified small chemicals that can be synthesized or purified and delivered as tablets or drinks.

These compounds mimic what happens in early embryos to reverse aging—but are potentially safer, simpler, and cheaper than gene therapy.

In mice, a four-week treatment makes skin and other tissues measurably younger. The same compound can even be applied topically to accelerate wound healing.

The Bigger Picture

What makes this approach fundamentally different from traditional anti-aging research is that it doesn’t just extend lifespan—it reverses biological age at the cellular level.

Sinclair’s vision extends beyond treating individual diseases to fundamentally changing how we think about medicine.

Instead of managing symptoms of aging-related diseases with multiple medications, what if we could restore youthful cellular function and let the body heal itself?

This represents a complete paradigm shift—from treating diseases as separate entities to addressing the underlying aging process that makes us vulnerable to them.

The eye trials beginning now will provide the first real-world evidence of whether this revolutionary approach can deliver on its promise in humans. If successful, it won’t just restore vision to people with glaucoma—it will open the door to treating virtually every age-related condition.
